The ACM application should include:
- The student's signature notarized
- Signature of Official of highest court (the Superior Court in the County where you live) you can obtain this information by calling your county courthouse and ask for the Superior Court Judge and they will direct you from there
- Information Sheet – completed with student's information including Social Security number
- Enclose your letter of acceptance showing the name of the degree major for which you have been accepted.
If any of the above information is missing or inaccurate there will be a delay in processing.
