Please note, for the forms, the only edits that should be made are for the project specific information such as number, name, amount, etc. No changes to the language (i.e. bond forms, affidavits, certificate of insurance) are allowed. Any revisions to such language must be submitted to the Director of Contracts & Services (sheree.srader@usg.edu) in writing for review and approval.

  This document should be used in the RFP process as a supplement to the proposal form found in Exhibit E. This will accompany the proposal as part of the QBS process as well as be attached to Exhibit E as listed in the final awarded contract.
