Records Retention Schedule Change Request Form

This form may be used by University System of Georgia personnel to submit a proposal for a NEW retention schedule, OR request a REVISION to an existing schedule. Your submission will be reviewed by the Regents Advisory Committee on Records Management. For more information see the Records Retention Schedule Change Request Process and Instructions.
