When it comes to keeping you and your family health, we’ve got you covered with these healthcare options:

Comprehensive Care - formerly known as the Open Access POS

  • Open Access Point of Service Network (OA POS)
  • You have a deductible to pay before the plan will pay for benefits
  • You can choose to go directly to a specialist
  • You pay a copay for doctors’ visits and urgent care
  • You pay coinsurance for outpatient surgery, hospital care, and specialized tests
  • Once you reach the annual out of pocket limit, the plan pays 100% of any remaining covered expenses

Consumer Choice HSA - formerly known as Health Savings Account (HSA) Open Access POS

  • Open Access Point of Service Network (OA POS)
  • You have a higher deductible to pay before the plan will pay for benefits
  • You have the option to have a Health Savings Account (HSA); Funds contributed to this account can be used during the current year or saved for future expenses
  • Employer contribution match $375 single coverage/ $750 family coverage
  • You can choose to go directly to a specialist
  • Once you’ve met your deductible, you pay coinsurance for healthcare services
  • Once you reach the annual out of pocket limit, the plan pays 100% of any remaining covered expenses

BlueChoice HMO

  • BlueChoice HMO network
  • There is no deductible on this plan
  • Your healthcare is coordinated through a primary care physician
  • You pay a copay for most services including doctors’ visits and services performed during visits, outpatient surgery, hospital care and therapies like physical, occupational and chiropractic care
  • There are no out-of-network benefits except in case of emergency
  • The plan pays 100% of any remaining covered expenses

Kaiser Permanente

  • Kaiser Permanente Provider Network
  • You will have your choice of physicians within the Kaiser network
  • Your healthcare needs are coordinated through a Kaiser Permanente primary care physician
  • Healthcare services are covered at 100% after paying your copay
  • You will receive personalized convenient care under one roof at various Kaiser Permanente locations

Please take the time to carefully weigh the plans available, and choose the option that’s best for you. Regardless of plan choice, make sure you take the time to complete the tobacco certification form. Without actively confirming you and any claimed dependents 18 and older are not active tobacco users, a tobacco surcharge of $75 per individual will be added to your premium every month. If you have questions, or need help, please contact your campus Human Resources/Benefits Office.

After 2015 Open Enrollment, November 3rd through November 14th, you will have to notify your Human Resources/Benefits Office to make any benefit changes as a result of Life Status Change or Family Status Change.

To locate Georgia providers for the BCBSGa Consumer Choice HSA, Comprehensive Care, or BlueChoice HMO plans:

  • Go to http://www.bcbsga.com/usg
  • Click on Find a Doctor
  • Click on Georgia Providers for Individual Plans and Group Plans
  • Under Plan Type, select POS or HMO from the drop-down menu and then Blue Open Access POS or BlueChoice HealthCare Plan to locate an In-Network Provider for the respective plans

To locate a provider in the Kaiser Permanente Network, for the Kaiser HMO plan:

  • Go to http://www.kp.org
  • Click on Find a Doctor
  • Select Georgia
  • Select or City or Zip Code
