USG Benefits

When it comes to keeping you and your family healthy, we’ve got you covered.

  • Consumer Choice HSA — Provided by Blue Cross Blue Shield of Georgia (BCBS), this plan was formerly known as the Health Savings Account (HSA) Open Access POS. This is the only healthcare plan that provides you with access to an HAS.
  • Moving to the Consumer Choice HSA for 2015? If you have an FSA now, make sure the balance is zero by December 31, 2014 — otherwise, you won’t be able to open an HSA until April 1, 2015.
  • Comprehensive Care — Provided by Blue Cross Blue Shield of Georgia (BCBS), this plan was formerly known as the Open Access POS, and it offers coverage for both in-network and out-of-network providers.
  • BlueChoice HMO — You receive benefits when your care is coordinated by your BCBS primary care physician.
  • Kaiser Permanente (KP) HMO — You receive benefits when your care is coordinated by your KP primary care physician.

What if I don’t enroll? If you have healthcare coverage for 2014 and do not enroll for 2015, your 2015 coverage will default to your 2014 coverage.

Please take the time to carefully weigh the plans available, and choose the option that’s best for you. Regardless of plan choice, make sure you take the time to complete the tobacco certification form. Without actively confirming you and any claimed dependents 18 and older are not active tobacco users, a tobacco surcharge of $75 per individual will be added to your premium every month. If you have questions, or need help, please contact your campus Human Resources/Benefits Office.

After 2015 Open Enrollment, November 3rd through November 14th, you will have to notify your Human Resources/Benefits Office to make any benefit changes as a result of Life Status Change or Family Status Change.
