Please distribute the following as needed. Action is required. KI0068 – Corrections to Foundation Table Changes FY07.02 SUBJECT: Corrections to Foundation Table Changes FY07.02. PURPOSE: To alert users of the corrections to the premium rate fields for the Employer portion. These Flat Rate Tables are tied to Disability Plans and total amount of premium should be keyed into the Employee Field. The Employer Field should be .00 zero. Flat rate tables affected are: KCD1, KCD3, KCD4, KCD5, KSD1, KSD3, KSD4, KSD5, RHD1, RHD3, RHD4, RHD5, RHM1, RHM2 and RHM3. FUNCTIONAL IMPACT: Go to the HRMS Website(http://www.usg.edu/gafirst/hrms/docs/ftc) to refer to the most current Foundation Table Changes dated 12/19/2006, 10:50 am(as noted in the footer of the document). Changes made to the spreadsheet have been highlighted in yellow. NOTE: Failure to update the foundation table with the correct rates will result in incorrect premium deductions.
